What is periodontitis?
- Periodontitis, also known as periodontal disease, has become one of the most common widespread diseases - more teeth are lost every year in Germany due to periodontitis than due to tooth decay.
- Periodontitis is a bacterial inflammation of the gums caused by bacterial plaque.
- If left untreated, it can lead to the destruction of the periodontium and, in the worst case, to tooth loss.
Remember: In the case of periodontitis, there is no pain or symptom to indicate that something is wrong. Even the famous bleeding gums often do not occur. Only the bad breath that now appears indicates the presence of a lot of bacteria. The only way to detect gum inflammation and the resulting gum recession is therefore to have it checked as part of a dental examination.

How does periodontitis develop?
Due to ineffective tooth brushing techniques, bacteria can accumulate on the periodontium and form a permanent biofilm (plaque), as is the case with caries.
After an initial simple gingivitis (inflammation of the gums), the bacteria penetrate the gums. This leads to a bacterial infection and thus to inflammation: periodontitis, or more precisely chronic periodontitis marginalis profunda. Many people also refer to the disease as periodontitis. The gums around the tooth are chronically inflamed.
This means that the inflammation does not hurt and is kept below a certain level by the body’s defenses. However, it progresses imperceptibly and the bacteria slowly destroy the connection between the tooth and gum, resulting in gum recession. The result is a periodontal pocket, a gap between the tooth surface and the surrounding soft tissue, filled with bacteria.
But that’s not all: after some time, this bacterial inflammation spreads to the underlying jawbone and destroys it. As a result, the tooth lacks support and becomes loose, even leading to tooth loss.

In the case of severe periodontitis, it may be necessary to remove teeth that would otherwise jeopardize the success of the therapy. Long-term treatment success is particularly important for this disease.

Investigation and planning
At the first appointment, the dentist will examine the entire oral cavity. An X-ray is usually also taken to assess bony structures.

If gum inflammation and possibly even gum recession are present, it is important to act decisively. The specially developed DR. Bacteria test and dental hygiene
From case to case, teeth that are no longer worth preserving are also removed during pre-treatment, as they hinder the success of this therapy.
In the next step, intensive dental hygiene is required. This is painless. The types of bacteria involved are also determined in this first step. For this purpose, a sample from several gum pockets is sent to a specialist laboratory in northern Germany.
The result allows us to determine the right antibiotic against precisely the harmful bacteria.
Therapy and medication
All teeth including the entire oral cavity are inspected. With the help of X-ray images and the innovative software, the DR. HAGER | dentists can create an exact analysis.
If a filling is required, the extent and position of the defect and the vitality of the tooth are examined in order to select the correct filling material.
A schedule with guaranteed fixed costs is then drawn up with the patient. Appointments can be booked afterwards or even carried out directly from time to time.
Review of success
As this therapy is also largely dependent on the patient’s cooperation, the success of the treatment is now evaluated. It may still be necessary to remove teeth that do not show the hoped-for success.
If compromises are made here, all teeth may become reinfected and everything starts all over again. However, success is usually achieved and patients are transferred to DH aftercare.
This means that patients who have been cured of periodontitis should attend dental hygiene appointments every 3-4 months during the first year to prevent a relapse during this very sensitive period. The teeth will thank you with a long life.

What is periodontitis?
This dental disease is a bacterial infection and inflammation of the tissues that hold the tooth in the jawbone. After a long period of time, the teeth are lost due to loosening as a result of bone loss.
Are there early signs of periodontitis?
This disease cannot be recognized in time by the layperson, as the inflammation of the gums is completely painless, cannot be seen and there are almost no signs of periodontitis. The famous “bleeding gums” sometimes do not appear at all or only appear relatively late, when bone damage is already present. In order to recognize periodontitis, you need a professional examination of the gums by a dentist.
How does periodontitis develop?
Due to inappropriate tooth brushing technique, large accumulations of bacteria adhere to the tooth-gum junction, forming a tough, sticky layer (“biofilm”) after a few days. The adjacent gums become inflamed by the bacterial attack and swell up, the connection to the tooth becomes permeable, a gap forms (“pocket”) and the bacteria multiply undisturbed in this pocket. Cell toxins and increasingly aggressive bacteria typical of periodontitis accumulate there. After some time, the underlying bone is also attacked and recedes. In the case of periodontitis, all these processes often go unnoticed for years and are – unfortunately – painless.
What is examined before starting periodontitis treatment?
Only a detailed examination for inflammation and gum pockets can reveal this gum disease. Laboratory tests, x-rays, determination of the location of bacterial plaques and determination of the tendency to bleed can contribute to a more accurate assessment of the severity of periodontal disease.
Is periodontitis dangerous?
International research is discovering more and more specific damage to other organs caused by periodontitis bacteria: heart attacks, diabetes, strokes, eye diseases, allergies, asthma, intestinal and kidney diseases, tendency to premature birth.

Are mouthwashes sufficient for periodontitis?
Unfortunately not. The bacteria and their hiding places must be completely removed using intensive manual cleaning techniques. Usually under specific treatment with antibiotics, the affected root surfaces are cleaned and smoothed with various instruments.
Anesthesia of the teeth is the rule. After careful disinfection, the gum inflammation heals completely very quickly.
Do loose teeth also need to be removed?
Very badly affected teeth cannot be cleaned effectively and must therefore be removed.
